Private Residence Supported Living
Supported Living residents live in their own home or apartment and receive services as needed or requested. Our staff support individuals to live as independently as possible while following person-centered support plans.
Activities of Daily Living
Our staff will support the individual in learning and practicing activities of daily living to gain and maintain independence. This may include support with hygiene, housekeeping, meal preparation, and more.

Home Environment
Individuals live in their own homes or apartments, creating a familiar and comfortable living environment. This setting encourages a sense of belonging and stability, which is crucial for overall well-being.
Socialization
Our Supported Living program prioritizes the health and safety of individuals. Our staff ensures that the living space is safe and accessible, and will provide necessary assistance in the case of emergencies.
Individuals have the opportunity to interact with staff, promoting social skills and reducing social isolation.
Independence
Our Goal
Individuals are supported to live as independently as possible in their own residence. Staff support is provided as needed or requested.
The goal of Supported Living is to support the individual to maintain their independence while residing in a comfortable and familiar environment.
24-Hour Support Home
Group Home residents live in a home provided by Branching Out with persons who have similar needs or interests. We provide a safe and supportive environment that encourages growth and development. Our 24/7 staff is dedicated to ensuring the well-being of our residents while fostering a sense of community and independence. We create and follow individualized, person-centered care plans to teach activities of daily living.

24/7 Staff Support
Socialization
Each Support Home has several residents with similar needs and interests. Individuals will have the opportunity to socialize with their housemates and gain a sense of community and belonging.
Health and Safety
Residents in our Support home will never be without staff. We provide 24-hour awake staff to assist the resident's day or night.
Medical Support
Branching Out supports all residents in the scheduling and attending of medical appointments. We also support residents in the self-administration of medication.
Regular deep cleaning by staff keeps our Support Homes in pristine condition to maintain the health and safety of our residents.
Our Goal
The goal for our 24-hour homes is to support residents in gaining the necessary skills to live independently, should that be what they choose.
